Financial Information

The Chicago Family Therapy Center is a Blue Cross Blue Shield Preferred Provider Organization (BCBS PPO). CFTC will submit claims for clients who have Blue Cross Blue Shield PPO insurance. Before you get started, we recommend that you check with your insurance provider about your coverage for behavioral health services.


For clients who have other health insurance providers, we ask that you pay for your sessions upfront. An invoice will be provided to you to submit a claim to your insurance provider for reimbursement. 


Individuals who do not have insurance may choose to pay our typical private pay rates as follows:

Initial Diagnostic Session (90791) $150-250*

*The first two appointments are initial diagnostic sessions*

Individual Psychotherapy (90837) $150-250 (60-minute session) 

Further payment options can be discussed when scheduling your first appointment.

What to expect

During your call, we will ask questions to match you with the therapist who will best meet your needs; including questions about your goals for care, availability and payment information. Please expect approximately 10 minutes for this call.



After your appointment is scheduled, you will receive an invitation to register for our Patient Portal. Once registered, you can review and sign the service agreement and fill out standard questionnaires we ask all new clients to complete before their first session.

Telehealth

Telehealth is a safe and convenient option for getting therapy at home. You may be able to attend appointments without needing transportation, taking time off work, or arranging for childcare. Those factors can be important in a long-term treatment plan. We conduct telehealth appointments via Zoom app on your computer, tablet, or smartphone. 


Prepare for your visit by finding a quiet, private place with a reliable internet connection. Privacy is an important part of feeling safe to share personal information with your health care provider. Even though telehealth appointments take place online, they are as private as in-person visits. If you can’t find a place for your appointment where you can be honest and open about your mental health, let your health care provider know.


Telehealth calls use your regular internet service or data plan. But, health care providers often use secure patient portals to message, call, and video chat. These connections are more secure than the typical internet.


Telehealth video appointments are not recorded. Your information is as private and safe as it would be if you were receiving in-person care.

Your therapist will send you a secure link through which to access your teletherapy appointment. Before your session, download Zoom by visiting zoom.us/support/download and install it on your computer or find it in your mobile or tablet app store.

Cancellation Policy:

We understand there will be unavoidable events that may prevent you from keeping your regularly scheduled appointments. However, we require a 24-hour notice in the event of a cancellation, unless in cases of sudden illness or emergency. Repeated violation of this policy or continued missed appointments on the client’s behalf may result in the client’s responsibility to pay that session’s full fee.
